If primary evidence cannot be provided to the office, then one is allowed to use secondary evidence to prove that they are legitimate United States citizens.

The United States passport application is also known as Form DS-11. There are two ways to complete Form DS-11, either on the government’s website, or by hand in the application office. Basic identification information will be asked on the form. The basic identification information includes name, contact information and a physical description of the applicant.

Fill out DS-11, the application form, and return it to the proper agency office. If you are applying in person, the officer will ask you to provide identification with your application. The identification that will be requested is either a previously issued and undamaged passport, a valid id card or driver’s license, a naturalization certificate or a military id or government id.

Cost

To expedite the process, a person’s application needs to be combined with the appropriate fees. In 2011, the price for an adult care and book costs one hundred sixty dollars. This figure comes from the execution fee costing twenty-five dollars added to the application fee which is one hundred and forty dollars.

Professional Photos

Applicants will need to provide a photo with their application. There are many requirements for the photo that need to be met. Among these requirements are size, quality and content. A photo must be in color and printed on photo quality paper. The applicant’s face should be in full view and the angle should be front and center. The entire photo should be two inches by two inches, with the head being at least an inch in size.
